Submit a Complaint

This form can be used to make a complaint about the way your case has been handled by the Complaints Investigation Scheme, or the conduct of the Aged Care Standards and Accreditation Agency and the conduct their officers in carrying out audits or making support contacts.

It is NOT a way to lodge a complaint about individual Aged Care service providers, nursing homes or facilities. Please refer all complaints of this nature to the Complaints Investigation Scheme on 1800 550 552.

We will only use the information you provide on this form for the purpose of dealing with your complaint. In order to deal with your complaint effectively, and depending on the nature of your complaint it may be necessary to disclose the information you provide on this form to the Department of Health and Ageing, which operates the Complaints Investigation Scheme OR the Aged Care Standards and Accreditation Agency.
